Working with your Health Insurance

We are currently in network with Blue Cross Blue Shield Plans of Texas. If you are in-network with a BCBS plan, we will bill your insurance for your individual sessions. Insurance often covers face-to-face time with a Registered Dietitian for both in-person and virtual appointments. Please note, any other services provided outside of one-to-one sessions are not eligible for insurance coverage nor considered a covered benefit.

Insurance coverage is mostly likely to cover individual sessions for diagnoses such as:

• Type 1 and Type 2 diabetes

• Pre-diabetes

• Cardiovascular disease and high cholesterol

• Hypertension

• Obesity

Plans often cover general wellness nutrition counseling without a specific diagnosis, particularly for preventive care benefits. This can vary significantly by plan, however.

Purely Remnant Wellness does not verify insurance benefits at this time. You must know and understand what your insurance covers, the deductible, co-payment, or co-insurance amounts. When you call, it is best to write down the date, the person you spoke with, and what they told you in case the claim is denied. You are responsible for the total amount of the services and will be charged with the credit card on file if insurance doesn’t cover the visit. 

For Nutrition Services: 

• Is nutrition counseling covered? (The procedure codes are 97802 and 97803)

• Are there any restricted diagnoses? (Ask specifically about the diagnosis code Z71.3 and if it's covered as preventive)

• Do I need a referral or prior authorization?

• Is nutrition counseling covered when provided via telehealth?

• How many visits are covered?

• Do I have a deductible to meet before insurance pays?

• Will I have a copay or coinsurance?

• Is Karen Ford (NPI 1295986941) covered under my plan? If not, what are my out-of-network nutrition benefits?

• Write down the date and reference number for your call.
